By SUBMITTING this ONLINE FORM, you will waive certain legal rights, PLEASE READ CAREFULLY.

IN CONSIDERATION of allowing myself AND/OR a minor/ward to participate in the programs, activities and events of CONDOR F.C and/or any of its affiliates, and/or leagues in which CONDOR F.C participates, I ASSURE YOU THAT:

I am the parent/guardian of the minor participant named above and have full legal responsibility for decisions related to the participant named above.
I believe that my child/ward is physically, emotionally and mentally capable of participating in the programs, activities and events of CONDOR F.C and/or its affiliates.
I hereby acknowledge that I am aware of the risks and dangers associated with or related to soccer.

Risks and hazards include, but are not limited to, injuries from:

Execute strenuous and demanding physical techniques in soccer.
Dryland training including weights, runs, and massage.
Grass, lawns, and other surfaces, including bacterial infections and skin rashes.
Falls to the ground due to uneven or uneven ground or surfaces.
Collisions with walls and soccer equipment.
Failure to properly use any piece of equipment or mechanical failure of any piece of equipment.
Extreme weather conditions that can cause heat stroke, sunstroke, or hypothermia;
Contact, collision or being hit by other participants, spectators, teams or vehicles.
Vigorous physical effort and strenuous cardiovascular exercises.
Exercise and stretch various muscle groups.
Travel to and from competitive events and associated non-competitive events that are an integral part of the organization’s activities.
In addition, I am aware that my child/ward may:
Suffering soccer injuries that can be serious, causing spinal cord injuries, and even be fatal.
Experiencing anxiety while challenging oneself during activities, events, and programs.
Coming into close contact with other participants, including the potential for accidental and health-related risks from unexpected contact with people who may carry infections/viruses.
The risk of injury is reduced if you follow all established rules for participation; and my risk of injury increases as I get fatigued.

I UNDERSTAND AND AGREE, on behalf of myself, my heirs, assigns, personal representatives and next of kin that my submission of this form to CONDOR F.C constitutes:

I am voluntarily registering my child/ward and my child/ward voluntarily participates in these activities, events and programs.
I accept that there are risks in soccer as described above and beyond and my child will be exposed to these risks and dangers.
I agree to accept all these risks and dangers and to be responsible for any injury or other loss that my minor/warded child may suffer while participating in these events, activities and programs.
If anything happens to my child, I release the Organizers from any liability for any claims, demands, actions and costs that may arise from my child’s participation. I understand “Organizers” to mean: CONDOR F.C., district associations, leagues, clubs and their directors, officers, members, employees, volunteers, officers, participants, clubs, agents, sponsors, facility owners/operators that CONDOR F.C. can use to execute its programs, and representatives.
I agree that if I decide to transfer myself and/or my child from the CONDOR F.C program to another club 30 days after submitting this registration for the current season, fees will not be refunded. $50.00 will be held if transferred prior to the 30 days listed above.

ACCIDENT INSURANCE

Execution of this agreement will not exclude you from accident insurance coverage, subject to the terms and conditions of the Ontario Soccer insurance policy.

IMPORTANT, READ BEFORE SENDING

WARNING NOTICE: There is potential risk in training for and participation in any sport and I understand all consequences thereof. CONDOR F.C has established rules for participation and proper conduct must be followed on or around the field of play.

RELEASE and WAIVER of Settlement of all claims:

By registering on this website with CONDOR F.C., I, the registrant and/or the registrant’s parent/guardian, agree to comply with the Published Rules.

The images and/or the text of the members of CONDOR F.C, including their names, may be used for internal and external advertising purposes of CONDOR F.C in various media.

I understand that as a registrant of CONDOR F.C., I may receive information from time to time related to soccer events, programs, and services.

I also accept and give permission for a member of CONDOR F.C. and/or a member of any of its affiliates to transport me and/or my child, if I am unable to do so myself, for any activity of CONDOR F.C. or its affiliates. affiliates. (Parent/Player/Executive) who is legally qualified to drive in Ontario releasing the driver, CONDOR F.C. and its members and affiliates from any liability.
I hereby assume all risks associated with my and my children’s participation with CONDOR F.C. and its affiliates and hereby release CONDOR F.C. and its members and affiliates, staff and executives, from any liability for injuries , losses or damages suffered or incurred. for me and/or my son in some way.

CONDOR F.C. reserves the right to cancel the membership only and when the member is found guilty of misconduct and/or when it does not follow the codes and/or rules established, its Teams and/or those organizations, Leagues Locals, de Renta its Teams.

I understand and agree to the above warning notice and release and waiver agreement, as well as the rules regarding registration fees by providing my own and my children’s information on this online form and that this document will be construed in accordance with the Laws of Ontario.

By registering with CONDOR FC through their online registration form found at www.condorfc.ca, I assume all responsibilities by submitting this form as if signing a printed copy.
